Transactional MFA Enforcement

What it does: Requires an extra MFA challenge when users visit specific pages/domains you list (step-up auth), not just at login.

How to set up:

1. Toggle ON.
2. Under TMFA working mode, select the challenge method (Authenticator or Email).
3. Click + Add Domain and enter a NAME and DOMAIN.
4. Use each row's ENABLE toggle to enforce per-domain; delete with the trash icon.
